  • Have you returned from overseas or another state in the last 14 days?

  • Have you been in contact with someone sent for COVID-19 testing or a confirmed case of COVID-19, other than at work?

  • Have you been in contact with someone sent for COVID-19 testing or a confirmed case of COVID-19 within the workplace?

Symptom check

  • A cough?

  • Any breathing difficulties?

  • A sore throat?

  • A runny nose?

  • Fatigue?

Temperature check

Is your temperature greater than 37.5 degrees?
